New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photographic ID.
You will need to complete a registration form (GMS1) and a health questionnaire which will be given to you when you call in to the surgery to register. This will provide us with useful information about you, whilst we wait for your medical records to arrive from your previous doctor. You can complete a form by clicking on the "Registering Online" tab. Completing this form does not guarantee you registration at our practice.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
We are currently accepting new patients
Because we hold a personal list service for our patients, new patients will be required to register with the doctor who is taking on new patients at the time of registration. This means if the doctor taking on new patients is based at Cheviot Road Surgery, the patient will be registered at that site.
